Near the end of the 20th century, the world returned to a dark age ruled by power after the nuclear war. The masters of the Hokuto, Nanto, and Gento Kō Ken martial arts have staked their own pride as they wage wars against each other. The stage has been set for an epic battle between these mighty men. Hokuto no Ken 5: Tenma Ryuusei-den Ai Zesshou is a role-playing video game produced by Toei Animation released on July 10, 1992 for the Super Famicom in Japan. It is the third and final RPG produced by Toei based on the Hokuto no Ken franchise, following Hokuto no Ken 3 and Hokuto no Ken 4, both for the Family Computer. Hokuto no Ken 5 features an original storyline, written once again by Hiroshi Toda with plot supervision by Buronson, set in an alternate universe from the original work's canon where a new hero must unite the warriors of the Hokuto, Nanto, and Gento schools to defeat a common enemy.

Fist of the North Star: 10 Big Brawls for the King of the Universe is a one-on-one fighting game for the Game Boy developed by Shouei System and originally released by Toei Animation in Japan on December 22, 1989. An American version was released by Electro Brain on April 1990, making it one of the few Hokuto no Ken video games released overseas. Other than a different title screen (which features a new artwork depicting Kenshiro and Raoh similar to the cover art of the first Famicom game), the two versions are otherwise identical.

Hokuto no Ken 2: Seikimatsu Kyuuseishu Densetsu is a side-scrolling action game for the Family Computer released by Toei Animation on April 17, 1987. It served as a tie-in to the anime series of the same name, being released a few days after the airing of its fifth episode. A version for the Nintendo Entertainment System was released by Taxan in North America under the title of Fist of the North Star on April 1989, making it the earliest Hokuto no Ken tie-in product released outside Japan. Before release, the NES version was shown in the Summer Consumer Electronics Show in 1988 under the title of Ken the Great Bear Fist, Toei's proposed localized title for the series. Because the game was produced at the same time as the Hokuto no Ken 2 anime series entered production, certain plot details differ from both, the manga and anime. Most notably, the renegade Gento Kō Ken masters from the anime, Taiga and Boltz, co-exists as enemy characters alongside Jakō's sons from the manga, Jask and Sheeno, while Bronza, a character exclusive to this game, serves as the General of the Red Light in place of Shoki, who filled the same role in the anime. While Shoki himself never appears in the game, his death at the hands of Sheeno is acknowledged in the official strategy guide. The game only covers the Tentei story arc, with a bonus stage featuring the Nameless Asura as a hidden final boss.

Up to five crystals can be collected per player to power up the Senshi's special attacks. During the most difficult moments in battle, Tuxedo Mask will appear and throw a rose, which is a bonus to pick up and depart.
