A follow-up to the first Spy vs Spy game now sees the spies stranded on a desert island. This time, instead of the four items which allow you to leave for the airport, the spies must try to collect 3 pieces of a missile. There are new traps and a single gun to take part in combat, rather than the big sticks of the previous game.

The black Spy and the white Spy are out to beat each other before the time bell rings. Find the needed objects by digging through drawers, closets and furniture in the house. Foil your opponent by setting creative booby traps in the various rooms (a bomb in a dresser drawer, for instance). Fights ensue when both spies enter the same room. Find hidden weapons in the rooms to help with those fights, since one of you will die if you run into each other.

Spy vs Spy: The Island Caper is a competitive multiplayer game where the goal is to recover all the vital items and leave before the other player does the same. It's a tropical-themed variant on the original. The Island Caper plays almost identically to the original Spy vs Spy game, just with a graphically updated island theme. Instead of rooms full of fixtures that items might be hidden behind, the player must search beaches on islands for items buried in the sand. Specifically, they need to assemble a missile from its parts, presumably so it can be launched at the opponent spy's home country. Finding the missile will allow the victorious spy to escape the island via a submarine (which inexplicably also includes an attractive woman). There are also traps which, like in the original, can be placed in spots where missile parts might be to fool opponents into killing themselves. The game was released on many of the home computers and consoles active at the time. Curiously, it also received a Japan-only NES port named "Nangoku Shirei!! Spy vs Spy" published by Kemco.

Foil your opponent by setting creative booby traps in the various rooms (a bomb in a dresser drawer, for instance). Traps can be disarmed with objects found in rooms (a water bucket from a firebox on the wall will disarm the bomb in the prior example). When all the items are together in the secret briefcase, head for the airport door.
